Welcome to the Avita Med Test, LLC website. By accessing or using this website or booking services through our website, you agree to be bound by these Terms of Service.

Services

Avita Med Test, LLC provides laboratory specimen collection, drug and alcohol testing, DNA testing, phlebotomy services, occupational health services, wellness testing, and related healthcare support services.

Availability of services may vary by location and applicable laws.

Appointments

Appointments may require:

• A non-refundable booking deposit
• Payment in full
• Approved Buy Now, Pay Later financing when available

Appointments are not confirmed until payment requirements have been satisfied.

Cancellations, No-Shows, Refunds & Service Credits

All appointment cancellations, rescheduling requests, deposits, refunds, service credits, and no-shows are governed by Avita Med Test, LLC's published Appointment Cancellation & Refund Policy, incorporated into these Terms and Conditions by reference.

No Refunds: All payments are considered final and non-refundable, except where required by applicable law or when Avita Med Test, LLC is unable to provide the purchased service and no mutually acceptable alternative is available.

Service Credits: Clients who cancel or request rescheduling with at least 24 hours' advance notice are eligible to receive a service credit equal to their eligible payment. Credits may be applied toward the originally purchased service or another available service of equal or greater value within ten (10) business days of the original appointment date.

Late Cancellations: Appointments canceled with less than 24 hours' notice may be subject to a $25 cancellation fee, deducted from any eligible service credit.

No-Shows: Clients who fail to attend their scheduled appointment without providing notice will forfeit their $25 appointment deposit. Any remaining prepaid balance may be issued as a service credit, subject to applicable fees and service restrictions.

Credit Conditions: Service credits are non-refundable, non-transferable, and have no cash value. Credits must be redeemed within the designated 10-business-day period. Services exceeding the available credit require payment of the difference.

Specialty Services: Services involving laboratory processing, specimen collection, special-order kits, or third-party vendor charges may be subject to additional restrictions once processing or preparation has begun.

Avita Med Test, LLC reserves the right to review documented emergencies and exceptional circumstances on a case-by-case basis while maintaining consistent and fair application of this policy.
